A testosterone panel that can't be interpreted is worse than no panel at all

The $39 single-marker test is the most common purchase in this category and the least useful. It produces a number that, on its own, frequently cannot distinguish a real problem from a normal result — and a false alarm here leads people somewhere expensive.

Short answer

A useful panel contains six markers: total testosterone, SHBG, albumin, free testosterone, LH and FSH — with estradiol and prolactin added depending on the picture. The draw must happen between 7 and 10am, because morning levels run 20–35% higher than afternoon in younger men. Avoid a carbohydrate-heavy meal beforehand: a high-glucose load acutely lowers total testosterone by 15–25% for several hours. And a single low result means repeat the test, not start treatment.

Why total testosterone alone tells you almost nothing

Most of the testosterone circulating in your blood is not available to your tissues. It is bound — tightly to sex hormone binding globulin (SHBG), loosely to albumin. Only the small unbound fraction, plus the weakly albumin-bound portion, is biologically active.

This creates a specific and very common failure. If your SHBG is high, a great deal of your testosterone is locked up, so total testosterone can sit comfortably in the normal range while the amount actually available to you is low. If SHBG is low, the opposite happens: a total testosterone result near the bottom of the range may correspond to a perfectly adequate free level.

Without SHBG and albumin, you cannot tell which of those situations you are in. That is why a lone total testosterone number is so often unresolvable, and why panels built around it sell so well — they produce a number, and a number feels like an answer.

Estradiol

Use the sensitive assay

Testosterone converts to estradiol via aromatase, and the balance matters. The important detail: standard estradiol assays are designed for female reference ranges and perform poorly at the low concentrations found in men. If estradiol is being measured in a male panel, it should be the sensitive (LC-MS/MS) assay — otherwise the number is unreliable.

Prolactin

When LH is low and testosterone is low

Elevated prolactin suppresses the pituitary signal and is a recognised, treatable cause of that pattern. It is a standard part of the workup when the picture looks secondary, and it is routinely missing from consumer panels.

Context markers

Thyroid, metabolic, iron

TSH, HbA1c or fasting glucose, a lipid panel, and ferritin. These are not testosterone markers, but they cover the conditions that most often cause the symptoms people attribute to low testosterone — and they change how SHBG behaves. If you are drawing blood anyway, they are the highest-value additions.

Testosterone has a pronounced daily rhythm. It peaks in the early morning and declines through the day, and in younger men the morning-to-afternoon difference reaches 20–35%. A 4pm blood draw can therefore return a "low" result in a man whose levels are entirely normal — and that false alarm is the beginning of an expensive and unnecessary road.

The fasting question, answered properly

Fasting is not technically required for testosterone. But eating before the draw is not neutral: a high-glucose load acutely lowers total testosterone by 15–25% for several hours, through insulin's effect on SHBG and suppression of LH. So while you won't invalidate the test by eating, you will lower the number.

In practice this is academic, because fasting for 8–12 hours is mandatory if the same draw includes a lipid panel or glucose — and those belong in any panel worth ordering. So: fast overnight, draw before 10am, black coffee is fine, no breakfast.

Other things that will move the number

Acute illness (including a cold), significant sleep deprivation, heavy alcohol the night before, and a hard training session in the preceding 24 hours all suppress testosterone temporarily. So does a period of high stress. If any of these applies, postpone the draw — a result taken under those conditions tells you about the week, not about you.

The pattern is consistent: the cheap panels omit precisely the markers that make the expensive marker meaningful. Paying $59 for an uninterpretable number and then $150 for the panel you should have bought is the common path. Where to order is covered in where to order your own blood work.

Reading the result: the two patterns

If total and free testosterone come back genuinely low on two separate morning draws, LH and FSH split the possibilities in two:

  1. Low testosterone, high LH and FSH — a primary pattern. The pituitary is signalling loudly and the testes are not responding. The signal is fine; the factory isn't.
  2. Low testosterone, low or inappropriately normal LH and FSH — a secondary pattern. The signal itself is weak, pointing upstream to the pituitary or hypothalamus. This is where prolactin, and causes like obesity, sleep apnoea, opioid use and significant stress, become relevant.

These lead to entirely different investigations. A panel without LH and FSH cannot tell you which one you're looking at, which is the clearest single argument for not buying the cheap version.

The sleep connection

Sleep restriction measurably lowers testosterone, and untreated obstructive sleep apnoea is a well-recognised contributor to a secondary pattern. Common questions

What time of day should a testosterone test be done?

Between 7 and 10am. Testosterone peaks in the early morning and falls through the day — in younger men, morning levels are 20–35% higher than late afternoon. Why isn't total testosterone enough?

Without SHBG and albumin you cannot tell which case you are in, so the total on its own is frequently uninterpretable.

What do LH and FSH add?

They locate the problem. Low testosterone with high LH and FSH means the pituitary signal is arriving but the testes aren't responding (primary). Low testosterone with low or normal LH and FSH means the signal itself is weak, pointing to the pituitary or hypothalamus (secondary). Completely different investigations follow, and no other marker distinguishes them.

Should I act on a single low result?

No. Testosterone varies day to day, and illness, poor sleep, alcohol and hard training all suppress it temporarily. Standard practice is to confirm with a second morning draw on a separate day. A single low number is a reason to repeat the test and see a doctor — not to start anything.

Can supplements raise testosterone?

Briefly: the evidence base for over-the-counter testosterone boosters is weak, the ingredient lists change faster than any research could follow, and correcting a genuine deficiency is a medical matter. If a panel shows a real problem, the answer is a clinician. If it doesn't, there is nothing to raise.
